Exceptional Family Member Program
The Exceptional Family Member Program is a mandatory enrollment program for all active duty personnel with family members with special needs. The goal of EFMP is to assist military families in managing the dual demands of a Navy career and the special needs of a family member. An exceptional family member includes a family member enrolled in DEERS who possesses a diagnosed physical, intellectual or emotional need that requires specialized medical or educational services. Enrollment in the EFMP program is designed to assist the sponsor with assignment to a duty station where appropriate services, necessary to support the family member(s), are available.

Identification Requirements - DOD has mandated that you MUST present your current, valid DOD ID card when you pick up your prescriptions. If you are picking up medications for someone else (other than a dependent child under 18) you must have their ID card, or a copy (both front and back of the card), this may be a digital image or paper copy.
Pharmacy Home Delivery
Skip the trip, with medications delivered to your home. Includes refills by mail, phone, or online; and an automatic-refill option. Active duty have no co-pay. For other patients, your co-pay is for a 90-day supply. To change your prescription to Home Delivery, visit Express Scripts, or download the Express Scripts app, or call 877-363-1303.

Refills
Call 877-211-1126, or refill online using the MHS GENESIS Patient Portal. You’ll need your prescription number (on the label) and the last four digits of the sponsor's social security number. Please allow 72 business hours for processing.
New or Renewal Prescriptions
- All prescriptions for non-controlled medications from off-base providers should be submitted electronically by your provider’s office to: DOD NBHC PORTSMOUTH NEW HAMP EPHCY (use 03904 zip code when searching).
- Prescriptions for CIII-V Controlled medications should be faxed to 207-438-2243 (prescription must physically be signed by prescriber).
Activating a Prescription
Call 877-211-1126, follow prompts for Portsmouth then select Option 4 to transfer to the pharmacy during normal duty hours. If voicemail picks up, speaking slowly and clearly leave the following information:
- Patient Name
- Patient Date of Birth
- Medication(s) that need filled
- Call back phone number including area code
- *** Pharmacy staff will return a call ONLY if there are questions ***
Prescriptions can also be activated via the MHS GENESIS Patient Portal.
TRICARE Retail Pharmacy Network

The Medical Home Port Clinic's primary function is to provide care to TRICARE Prime and designated SPP (Special Provisions Patient) beneficiaries. Other categories of patients are seen on a space available basis. TRICARE Prime patients are enrolled to a specific Primary Care Manager. In the event that an appointment is not available with the assigned Primary Care Manager, another provider on the team may provide the needed services. Appointments are required for all services except for PRIME patient walk in protocols.

Contraceptive Services
Beneficiaries wanting contraceptive services are referred to a TRICARE network provider to ensure our patients have options for the full range of contraceptive care.
Our nursing team will triage care that we can provide in-house, which is limited due to personnel and training constraints.
Walk-In Contraceptive Services
A same day walk-in service providing immediate contraceptive access, sexual transmitted infections testing, sexual health & family planning education, and PrEP to optimize warrior readiness. Open to all Active duty and eligible beneficiaries on a space available basis on Wednesdays from 9 - 10 a.m. at the NMRTU Portsmouth MHP clinic. **This is a first-come, first-served clinic for beneficiaries ages 16 and older**

- Emergency Contraception (Plan B One-Step® - can go directly to Pharmacy)

Services We Don’t Provide
- We do not treat major emergency medical conditions, such as chest pain, shortness of breath, large burns, open fractures, loss of consciousness, large lacerations, uncontrolled bleeding, etc. Please activate the local emergency response for these conditions @ x2333.
- We do not prescribe medications or administer medications here in the clinic.
How to Make an Appointment
Preplacement, periodic or termination exams:
These appointments are scheduled through Code 1102 based on job position requirements. Please bring any required forms with you to your visits, including Respirator Forms completed by your supervisor. When checking in, you will be required to present your CAC card or your social security number for identification. Of note, badge numbers are not a valid ID at the clinic.
If you are unable to make your scheduled appointment, please call the clinic at x5981 (207-438-5981) prior to your appointment. If you are more than 15 minutes late for your appointment, it will be canceled, and you will have to reschedule. No exceptions.

Unscheduled medical care:
For minor injuries that occur on the shipyard such as superficial abrasions or lacerations, minor punctures, and sprains, please come directly to the clinic for evaluation and present yourself to the front desk. Feel free to call ahead to see if your injury is appropriate for our clinic. If you are in need of urgent or emergent medical care, please call emergency response at x2333.
Audiology and Hearing Conservation
The Audiology Department provides Hearing Conservation services to civilian shipyard workers, and all Active Duty, National Guard, and Reserves. All civilian appointments are scheduled by Code 1102 (shipyard medical exam scheduling). For active duty, National Guard, and Reserves, services are provided on a walk-in basis for the audiology screening booth or via appointment with the Audiologist if a full diagnostic evaluation is required.

Optometry & Vision Screenings
The Optometry Department offers the services below. Of note, we do not have an Optometrist on staff so we cannot provide new vision prescriptions. For any services requiring a vision prescription, you must obtain a current (within a year) prescription from an outside Optometrist. For service members, please request a referral to an optometrist through your primary care provider.

- Vision screenings for PNSY civilian employees (scheduled through Code 1102)
- Walk-in
- Vision screening for active duty personnel. Please bring completed medical forms to your vision screening exam.
- Order eyeglasses for active duty personnel and Retirees. Please bring your current (within 1 year) vision prescription and eyeglasses (if worn) to your visit.
- Safety Glasses Program for PNSY active duty personnel and PNSY civilian employees. Please bring current (within 1 year) eyeglass prescription and a completed authorization form from your supervisor. We can do small repairs depending on the issue. Services available Monday through Thursday, 8-11 a.m. & 1-3 p.m. (contingent upon staffing)
To make your Optometry visit run more smoothly, please do the following:
- Bring your current (within 1 year) prescription eyeglasses, readers or contact lenses to your appointment.
- Bring your authorization forms signed by your supervisor and a current (within 1 year) vision prescription if ordering any type of eyeglasses or safety glasses.
